LIABILITY WAIVER & RELEASE — PLEASE READ

ASSUMPTION OF RISK: I understand that participation in fitness activities, use of gym equipment, and presence at Iron Forged Gym involves inherent risks of serious physical injury or death. I voluntarily assume ALL such risks.

RELEASE OF LIABILITY: In consideration of being allowed access to Iron Forged Gym, I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Iron Forged Gym, its owners, directors, officers, employees, agents, and assigns from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury (including death) that may be sustained by me while on the premises or traveling to or from the facility, whether caused by negligence or otherwise.

24-HOUR ACCESS & CONDUCT: I agree to follow all posted rules. I will not share access codes, tailgate, spar, or engage in any unsafe behavior. I understand gym staff may not be on duty at all times.

BOXING AREA: Absolutely NO sparring, fighting, wrestling, grappling, or contact sports of any kind are permitted anywhere on the premises.

EQUIPMENT: I agree to use all equipment as intended. If I observe damaged or unsafe equipment I will immediately stop use and notify the gym by phone (724-550-4595). I understand I am financially responsible for any willful damage I cause.

MEDICAL: I certify that I am in good physical health and am not aware of any medical condition that would make my use of the gym unsafe.

ARBITRATION: Any dispute arising from my use of Iron Forged Gym shall be resolved exclusively by binding arbitration in Fayette County, Pennsylvania. I waive my right to a jury trial.

ACKNOWLEDGMENTS

I understand that participation in fitness activities involves risk of serious injury or death, and I voluntarily assume all such risks. I agree to release, hold harmless, and indemnify Iron Forged Gym, its owners, employees, and agents from any and all claims, damages, or liability. I agree to follow all posted gym rules, including proper use of the key fob/access system and the no-tailgating policy. I agree that willful damage to equipment will result in removal from the facility and I will be responsible for repair/replacement costs. I confirm that I am at least 18 years of age (or have obtained parental/guardian consent if under 18) and that all information I provided is accurate.
